This is a chart.

The header/top row goes across the chart and contains the following:
Headline in top left-hand corner: ‘What do I think?’
On the top right of the row: ‘Name:’ and space for this to be entered and underneath: ‘Date:’ and space for this to be entered.
The instruction going across the header/top row: ‘Please tick in the boxes below what you think you are like at Managing Yourself.’
Below this instruction, and in the middle third of the chart (above the middle column described below), there are the words: ‘Before C. Games Unit’.
Beside this, and in the right-hand third of the chart (above the right-hand column described below), there are the words: ‘After C. Games Unit'.

Below the header/top row, there are three main columns – all the same width, and together spanning the width of the table.

The column on the left is headed: ‘Managing Ourselves’. Underneath this heading, there are the following rows:
‘Making sensible choices for ourselves.’
‘Be responsible for our own actions and behaviours.’
‘Look after our own property.’
Set realistic goals and say how we will achieve them.’
‘Plan what to do and stick to our plan.’
‘Get tasks finished on time.’
‘Organise ourselves independently.’
There is a double line separating this first column and the second column.

The column in the middle is split into four mini-columns. It sits under the wording in the header/top row of: ‘Before C. Games Unit’. On the same row as the ‘Managing Ourselves’ heading in the left-hand column, there are headings for these four mini-columns: starting from the left-hand side, these read:
‘A strength’
‘If I try hard’
‘Not always’
‘Hardly ever’
After this fourth mini-column there is a double line separating the second and third column.

The column on the right is also split into four mini-columns. It sits under the wording in the header/top row of: 'After C. Games Unit’. The four mini-columns have the same headings as those in the middle column.

The other cells in the chart are blank, except for the wording that is superimposed in the centre:
‘Using different coloured pens, this self-assessment sheet was completed by each student, a buddy, and a teacher. Following this, a conference was held with the teacher.’
